Cardiac imaging in pulmonary embolism: assessment of right ventricular dysfunction by tissue Doppler.

چکیده
O tromboembolismo pulmonar (TEP) é uma entidade relativamente frequente no contexto de urgência hospitalar, com uma elevada taxa de mortalidade e morbilidade quando não tratado precocemente, continuando a representar um desafio diagnóstico na prática clínica diária, sendo a sua apresentação clínica nem sempre esclarecedora, especialmente no contexto agudo1. A mortalidade de doentes por TEP nos quais o diagnóstico não é efetuado tem sido reportada como sendo quatro a seis vezes superior à dos doentes identificados e adequadamente tratados (mortalidade intra-hospitalar de 6-8% versus mortalidade global TEP não tratado 25-30%)2,3. A maior dificuldade no tratamento eficaz do TEP reside no seu diagnóstico atempado. A apresentação clínica por vezes impercetível de TEP maciço, a necessidade da utilização de diversas técnicas de diagnóstico e a não existência de um exame gold standard de fácil acesso e interpretação, são algumas das causas que explicam a dificuldade conhecida e o desafio no diagnóstico. No último Congresso Europeu de Cardiologia foram apresentadas as mais recentes Guidelines no diagnóstico e manejo do TEP da Sociedade Europeia de Cardiologia.
